Seborrheic Dermatitis presents as moist, transparent to yellow, greasy scaling papules among red patches and plaques. Seborrheic Dermatitis usually favors areas where the concentration of sebaceous glands is maximal such as the scalp margins, central face and presternal areas, Characteristic locations are the eyebrows, the base of eyelashes, nasolabial folds and paranasal skin, and external ear canals. Scale adherent to the eyelashes and lid margins with variable amounts of erythema is characteristic of seborrheic blepharitis. Flexural skin may be similarly involved including the postauricular, inguinal, and inframammary folds as well as the anogenital area. Adults tends to have a chronic course with remissions and exacerbations. Flares are precipitated by stress, fatigue, and seasonal climate change.</p>

It has been proposed that Pityrosporum yeast is the cause. All ages can be affected though the clinical presentation among children differs from that of adults. For many patients, seborrheic dermatitis is primarily a cosmetic issue. Patients are bothered by facial redness or showers of dandruff on dark colored clothing. For some patients, seborrheic dermatitis is a manifestation of another underlying process. Seborrheic dermatitis has greater severity and is more difficult to control in patients with neurologic disease such as head trauma, spinal cord injury, Parkinson&#8217;s disease or stroke. Patients with human immunodeficiency virus (HIV) infection are often afflicted with severe recalcitrant flares.</p>

Mild to moderate facial seborrheic dermatitis may respond well to topical antifungal creams or gels (Loprox) applied daily. Daily facial washing with antidandruff shampoo or soaps containing zinc pyrithione or selenium sulfide is also effective. Group VI or VII topical steroid creams or lotions applied bid for several days may be required periodically for control. Off-label use of Protopic 0.1% ointment or Elidel 1% cream applied daily are effective long-term alternatives to topical steroids. Mild to moderate scalp involvement is best managed with frequent and extended shampooing with antidandruff shampoos. Effective formulations may contain ketoconazole coal tar, salicylic acid, selenium sulfide, zinc pyrithione, or sulfur. Dense, thick, adherent scale is removed by applying warm mineral or olive oil to the scalp and washing several hours later. At bedtime apply 10% liquor carbonis detergens (LCD) in Nivea oil or Derma-Smoothe FS lotion (peanut oil, mineral oil, fluocinolone acetonide 0.01%) to the scalp and wash out in the morning. Wetting the scalp prior to application and using a shower cap will help penetration. Blepharitis may be suppressed by lid massage and frequent washing with zinc- or tar-containing antidandruff shampoos. Severe cases may require oral antifungal therapy to eradicate Pityrosporum ovale.
